HMO.
Rather, it is a collection of healthcare providers who agree to submit claim forms
on your behalf and accept an agreed-upon payment so you do not acquire any surprise
bills.
You will be responsible for any remaining deductible or coinsurance outside of what
the plan pays for Eligible Charges.
